Cause of chest pain

This lady presented with atypical chest pain and shortness of breath.


The CXR showed an opaque shadow over the right heart border.


The CXR was repeated after a NG tube insertion.
Guess what it was?

The NG is in the chest, ?oesophageal perforation and mediastinitis.
